Intraoperative PDT of malignant supratentorial tumors
Abstract
Unsatisfactory results of treating the malignant tumors of the brain induce the necessity of developing new technologies of treatment.
In A.L. Polenov's Research Institute of Neurosurgery since 2002, PDT technology has been developed in neuro-oncology with an assessment of its safety and effectiveness. PDT was performed in the treatment of 80 patients with malignant gliomas (grade III and IV) (2002–2019). Since 2023, the use of intraoperative PDT has been started in patients with secondary brain tumors (n=12) and in patients with recurrent anaplastic meningiomas (n=7), the effectiveness of intraoperative PDT was investigated by studying morphological changes in the perifocal zone. The level of expression of the proapoptotic marker caspase-3 was assessed, and control MRI data were studied, (assessment of the perifocal zone in the mode Т2-WI, FLAIR, DWI), as well as assessment of the clinical and functional status of patients.
Due to its high selectivity of action, PDT is a promising technology compared to classical treatment methods used in neuro-oncology. The data obtained indicate a positive therapeutic effect of intraoperative PDT in the structure of complex treatment of patients with malignant tumors of the central nervous system.
